The primary purpose of the Central Intake & Case Management Coordinator position is to facilitate the intake, referral, and admission process for assigned skilled nursing facilities while serving as a liaison between referral sources, hospitals, managed care organizations, residents, families, and facility teams.

Referral Management Functions
