Home Choice MI, LLC is a consumer-directed home care agency dedicated to helping individuals live independently through Michigan's Home Help Program. We're growing our team and looking for an experienced Intake Specialist to join us.

Position Summary:

We're seeking an Intake Specialist with hands-on experience in Michigan's Home Help program to manage the client intake process from initial referral through enrollment. The ideal candidate understands the Home Help application process, MDHHS requirements, and consumer-directed care model, and can guide clients and families through enrollment with compassion and efficiency.

Responsibilities:

  • Process new client referrals and intake applications for the Home Help Program

  • Guide clients/caregivers through eligibility verification and required documentation

  • Liaise with MDHHS caseworkers and coordinate paperwork submission

  • Maintain accurate, compliant client records

  • Communicate program requirements clearly to clients and families

  • Track intake pipeline and follow up on pending applications

Requirements:

  • Prior experience with Michigan's Home Help Program intake process (required)

  • Familiarity with consumer-directed home care / Medicaid waiver programs

  • Strong organizational and communication skills

  • Ability to work independently and manage a caseload

  • Located in or near Oakland or Macomb County
