Remote -- must reside local to Erie/Crawford County PA
A RNAC coordinates the MDS and Care Planning process to ensure timely and accurate submission with the Interdisciplinary team.

Responsibilities

  • Assist with nursing staff education in the documentation, MDS, CAA's and Care Plan process
  • Must accurately complete MDS assessments in accordance with established time frames for new admissions, quarterly, annual and significant change
  • Must precheck and electronically submit MDS’s to State agency, review validation reports and make necessary changes
  • Coordinates the scheduling and timely completion of MDS’s and Care Plans with the Case Manager and Interdisciplinary team
  • Participates in the QAPI process and the “Nurse On-Call” program
  • Able to electronically access, print and evaluate Quality Indicators

RequirementsGraduation from an accredited school of Nursing. Current license as a Registered Nurse with the State Board of Nursing. CPR certification recommended. A Licensed Practical Nurse may practice as a MDS/RNAC under the direction of an RN within the state of Ohio.
