• Conduct comprehensive clinical assessments of pediatric patients, contributing to individualized care plans that address their emotional and psychosocial needs.

• Utilize evidence-based therapeutic interventions, including solution-focused therapy, to support children and their families through challenging circumstances.

• Maintain meticulous clinical records and documentation, ensuring compliance with state regulations and organizational policies.

• Collaborate closely with medical professionals, nurses, and multidisciplinary teams, ensuring an integrated approach to patient care.

• Engage in regular case consultations and peer supervision meetings to strengthen care quality and professional growth.

This role primarily focuses on pediatric patients in hospice care, their siblings, and caregivers, addressing the unique emotional and psychological challenges related to chronic illnesses and end-of-life care. Your expertise in social work will help families cope with grief, provide resources for support, and ensure that children's voices are heard in their care.

• A Master’s degree in Social Work from an accredited institution.

• Active LCSW license in the state of Pennsylvania.

• A minimum of three years of relevant clinical experience, particularly in pediatric care or hospice settings.

• Strong proficiency in solution-based therapeutic interventions and commitment to best practices.

• Exceptional communication, advocacy, and interpersonal skills, able to engage effectively with patients, families, and colleagues.

• Ability to thrive in a remote work environment, demonstrating self-motivation and effective time management.

Experience in hospice or palliative care settings is highly desirable, as is familiarity with the emotional and social aspects of caring for children with serious illnesses. Skills in crisis intervention and trauma-informed care will also be beneficial in navigating the complexities of this work.
