• Conduct in-depth clinical assessments to understand individual patient needs and develop tailored treatment plans that promote coping strategies and emotional resilience.
• Utilize evidence-based therapeutic interventions and solution-focused practices to guide patients in managing their mental health challenges effectively.
• Maintain thorough, accurate, and timely clinical documentation following legal and ethical standards to support patient care and regulatory compliance.
• Collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and allied health staff to ensure a holistic approach to patient care.
• Offer education and ongoing support to patients and their families, helping them to understand the complexities of mental health issues and the resources available to them.
• Engage in continuous professional development, staying informed of the latest research, treatment modalities, and best practices in mental health care.

In this role, you will work with a diverse range of patients, including individuals facing serious illnesses and their families. Your expertise will not only support those navigating the emotional aspects of terminal diagnoses but will also provide guidance through the complexities of end-of-life care. You will play an essential role in fostering an environment of dignity, respect, and understanding for all involved.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Pennsylvania is mandatory for this position.
• Demonstrated knowledge and experience with solution-focused therapy techniques and other evidence-based interventions.
• Strong communication and interpersonal skills to effectively engage with patients, families, and colleagues.

• Previous experience in a hospice or palliative care setting is highly desirable.
• Familiarity with electronic health record (EHR) systems to facilitate appropriate documentation and treatment plan updates.
• Experience collaborating in a multidisciplinary treatment team to optimize patient outcomes.
