• Conduct comprehensive bio-psychosocial evaluations to identify barriers to treatment and develop tailored care plans that enhance patient outcomes.
• Collaborate closely with a multidisciplinary team, including medical and support staff, to devise integrated treatment strategies and improve care coordination.
• Utilize evidence-based therapeutic modalities, both individually and in group settings, to facilitate the healing process for clients.
• Document social work activities and patient interactions in compliance with departmental standards, ensuring accuracy and confidentiality of all records.
• Educate patients and their families about their health conditions, treatment options, and the importance of adhering to recommended care pathways.
• Advocate for patients' needs within the community, focusing on empowering clients through access to social support and resources.
• Maintain knowledge of available community resources and assess their applicability to referred patients.
• Participate in department meetings and training opportunities to enhance professional skills and clinical knowledge.

This role primarily serves children, adolescents, and families within school settings. As an LCSW, you will address a wide range of psychosocial issues stemming from emotional, behavioral, and developmental disorders. Your interventions will focus on enabling clients to navigate their challenges effectively, thereby improving their academic and social functioning.

• A Master’s Degree in Social Work from an accredited institution is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Solid understanding of the NASW Code of Ethics and adherence to professional standards of practice.

• 1-2 years of clinical experience in a mental health or health-based setting, particularly with youth and families.
• Demonstrated training and experience working with diverse populations, understanding varying age-related developmental challenges.
• Proficiency with telehealth technologies and electronic health record systems.
