• Conduct thorough psychosocial assessments to identify patients’ emotional, social, and financial needs, ensuring that care plans are comprehensive and tailored to individual circumstances.
• Provide individual and group counseling sessions to patients, addressing complex issues, including adjustment disorders, grief, depression, and coping mechanisms.
• Actively collaborate with community organizations to coordinate necessary post-discharge services for patients, facilitating smooth transitions and continuity of care.
• Offer education and emotional support to patients and families about treatment options, available resources, and care management strategies.
• Participate in hospital committees, lending expertise on social work practices and advocating for policies that enhance patient care.
• Deliver crisis intervention services as necessary, maintaining a calm and supportive environment for patients and families experiencing distress.
• Work with interdisciplinary teams to refine patient care plans and to optimize healthcare delivery, improving overall patient outcomes.
• Document all interactions, assessments, and treatment plans meticulously to adhere to hospital policies and legal standards.

This position particularly focuses on patients in hospice care, who may be facing end-of-life issues, chronic pain, or terminal diagnoses. Your work will involve not just patients, but also their families, many of whom may be experiencing significant emotional distress. You will be instrumental in helping both patient and family navigate the complexities of grief, loss, and decision-making during this critical time.

• A Master’s degree in Social Work from an accredited institution is required.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Virginia is mandatory.
• Proficient understanding of medical terminology and treatment modalities pertinent to hospice care is essential.
• Strong communication and interpersonal skills are critical to build relationships with patients, families, and multidisciplinary teams.

• A minimum of two years of experience working in a healthcare or hospital setting is preferred. Experience within hospice care or palliative support is a distinct advantage.
• Familiarity with crisis intervention techniques and therapeutic practices is highly beneficial.
