• Conduct thorough psychological and social assessments of patients to develop personalized treatment plans that address their emotional and social needs.
• Implement evidence-based therapeutic interventions, adjusting approaches as needed based on patient progress and feedback.
• Regularly document patient interactions, treatment plans, and progress notes in compliance with confidentiality standards and organizational policies.
• Engage in interdisciplinary team meetings to discuss patient status and adjust care plans in collaboration with healthcare providers.
• Stay abreast of current research and best practices in the field of social work and hospice care, actively integrating new knowledge into practice.

Our primary focus is on serving individuals experiencing advanced chronic illnesses and their families. As an LCSW at Renewed Path Counseling, you will engage with patients from diverse backgrounds, helping them and their loved ones navigate emotional distress and grief associated with terminal diagnoses. You will also support family members in exploring coping mechanisms and making informed decisions about end-of-life care.

• A Master’s degree in Social Work from an accredited institution.
• An active LCSW license in the state of Florida.
• Exceptional communication skills, both verbal and written, to effectively interact with patients, families, and team members.
• Strong analytical and problem-solving abilities to assess situations and develop appropriate interventions.

• Previous experience working in hospice or palliative care settings is highly desirable.
• Exposure to trauma-informed care models and techniques.
• Familiarity with teletherapy platforms and electronic health record systems.
