• Conduct in-depth clinical assessments to understand patients' mental health needs and develop customized treatment plans.
• Administer evidence-based therapeutic interventions, including solution-focused therapy and cognitive-behavioral strategies, to support patients’ mental health improvement.
• Maintain accurate and timely clinical documentation in accordance with healthcare regulations and organizational policies.
• Collaborate effectively with a multidisciplinary treatment team to ensure comprehensive care delivery and continuity of services.
• Educate and support patients and their families in navigating challenges related to serious illness, fostering resilience and understanding.
• Stay informed about the latest research and advancements in mental health care to enhance your practice and improve patient outcomes.

In this role, you will work primarily with individuals facing serious health challenges and their families. By providing empathetic counseling and supportive interventions, you will guide patients through a difficult life stage, addressing both mental health needs and practical concerns. Your work will directly contribute to improving patients' overall quality of life, helping them find peace and comfort during challenging times.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Florida.
• Comprehensive knowledge of evidence-based therapy interventions and solution-focused approaches.
• Strong communication and problem-solving skills, with a demonstrated ability to build rapport with diverse populations.

• A minimum of 2 years of relevant social work experience, ideally within hospice, palliative care, or a similar healthcare setting.
• Experience working with interdisciplinary teams, contributing to holistic patient care.
• Familiarity with electronic health record (EHR) systems to streamline documentation and care coordination.
