• Conduct comprehensive assessments of patients' mental health, evaluating emotional, behavioral, and social functioning to identify treatment needs.
• Provide evidence-based psychotherapy and counseling services tailored to individuals coping with mental health conditions such as anxiety, depression, and PTSD.
• Design and implement personalized treatment plans that are reflective of each patient's unique cultural background and specific challenges.
• Collaborate closely with interdisciplinary teams, including psychiatrists and primary care providers, to ensure cohesive and effective patient care.
• Monitor patient progress in therapy, adjusting treatment plans as needed based on patient feedback and response to interventions.
• Educate and empower patients and their families regarding mental health issues, available treatment options, and local community resources.
• Maintain strict adherence to ethical standards and professional guidelines, ensuring confidentiality and informed consent in all patient interactions.

The LCSW will primarily work with patients facing complex mental health challenges in a hospice environment, focusing on those requiring end-of-life care. You will play a vital role in helping patients navigate their experiences and find peace during often challenging times.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of 2 years of experience in providing mental health services, particularly in a clinical or hospice setting.
• Exceptional communication and interpersonal skills to effectively build trusting relationships with patients and collaborate with diverse healthcare professionals.
• Demonstrated ability to develop evidence-based, individualized treatment plans and conduct thorough psychological assessments.
• Proficient in electronic health record (EHR) systems for documenting and managing patient care.

• A deep commitment to confidentiality, ethical practice, and patient advocacy.

• Experience in hospice care or palliative settings is highly desirable.

• Background in community resource navigation for patients and families affected by mental health issues.
