As a Licensed Master Social Worker (LMSW) at Gotham Enterprises Ltd, your day-to-day responsibilities will include:

• Conducting thorough biopsychosocial assessments to understand patients’ needs, preferences, and goals for care.
• Developing and implementing individualized treatment plans that are grounded in evidence-based practices and tailored to the unique circumstances of patients encountering end-of-life challenges.
• Providing emotional and psychological support to patients and families, helping them navigate complex feelings and challenges during the hospice journey.
• Facilitating access to community resources and services to support patients' needs, including palliative care options, bereavement services, and financial assistance programs.
• Documenting all interactions and interventions accurately in electronic health records while adhering to HIPAA regulations.
• Participating actively in multidisciplinary team meetings, case consultations, and peer supervision to ensure quality care and continuous improvement in service delivery.
• Advocating for patients’ rights and preferences in care discussions, providing education on available treatment options and community supports.

You will primarily serve patients with life-limiting illnesses and their families, focusing on those in hospice care. This includes engaging with diverse populations who may be experiencing emotional distress, grief, and uncertainty. Your role will be vital in promoting dignity, comfort, and peace during this sensitive time.

• A valid state license to practice as a Licensed Master Social Worker (LMSW).
• A Bachelor's degree in Social Work from an accredited institution is required; a Master's degree in Social Work (MSW) is highly preferred.
• A minimum of three years of experience in a clinical social work setting, preferably within hospice or end-of-life care.
• Strong understanding of psychosocial assessments, interventions, and treatment methodologies relevant to hospice and palliative care.
• Excellent interpersonal and communication skills, with a demonstrated ability to empathize with patients and families.

• Experience working in hospice, palliative care, or end-of-life settings.
• Familiarity with electronic medical record systems and telehealth platforms.
• Knowledge of local community resources for hospice and bereavement support.

The salary range for this position is competitive, offering between USD 80,000 and 100,000 annually, commensurate with experience.
