• Conduct thorough assessments of clients to determine their specific needs and create personalized treatment plans that address their unique circumstances.
• Implement evidence-based therapeutic interventions, ensuring that clients receive the highest standard of care tailored to their individual backgrounds and challenges.
• Maintain precise and confidential records in accordance with regulatory standards, ensuring all documentation is current and accurate.
• Collaborate with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to coordinate comprehensive care for patients.
• Stay abreast of the latest research and advancements in social work, applying innovative approaches to care as appropriate.

This position focuses primarily on clients receiving hospice care, including patients with terminal illnesses and their families. You will provide emotional support, counseling, and guidance during what can be one of the most challenging times in their lives. Your role will involve not only treating clinical issues like anxiety and depression but also addressing psychosocial stressors that affect overall well-being. Each interaction seeks to empower clients and provide them with the compassion and dignity they deserve.

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of a current LCSW license for the state of New Jersey.
• Strong communication skills, both verbal and written, with exceptional ability to engage with clients empathetically.
• Experience in a hospice or palliative care setting is preferred but not mandatory.
• Proficient in the use of electronic record-keeping and remote communication tools.

While not required, candidates with prior experience in hospice care, trauma-informed practices, or crisis intervention are encouraged to apply. An understanding of how socio-economic factors affect mental health and the ability to advocate for resources will be beneficial in this role.
