• Conduct comprehensive assessments to evaluate patients' emotional, social, and psychological needs in order to develop personalized treatment plans.
• Utilize solution-focused therapy and evidence-based practices, including cognitive behavioral therapy and bereavement support, to assist patients and families in coping with end-of-life issues.
• Maintain accurate and timely documentation of patient records and treatment outcomes in compliance with organizational policies and regulatory standards.
• Collaborate with a multidisciplinary team, including physicians, nurses, and spiritual care counselors, to provide comprehensive care plans.
• Educate patients and families about available resources, coping strategies, and the dying process to enhance understanding and promote peace.
• Keep abreast of current trends in hospice care and mental health practices through ongoing training and professional development opportunities.

This role focuses on serving patients in the hospice setting, where you will address the unique needs of individuals nearing the end of life. You will provide emotional support and counseling to both patients and their families, assisting them in navigating the complexities of grief and loss. Your interventions will play a crucial role in enhancing the quality of life for our patients, facilitating meaningful conversations and decisions about care preferences and advanced directives.

• A Master’s degree in Social Work from an accredited institution, equipping you with the foundational knowledge and skills necessary for this role.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of New York is required.
• Demonstrated knowledge and proficiency in various therapeutic techniques, particularly solution-focused and cognitive-behavioral interventions.
• Exceptional communication and interpersonal skills, along with the ability to build rapport with patients and families from diverse backgrounds.

• At least two years of experience in a clinical setting, particularly in hospice, palliative care, or end-of-life counseling.
• Experience working with interdisciplinary teams in healthcare settings, demonstrating an understanding of the collaborative process essential for effective patient care.
• Familiarity with telehealth platforms and electronic medical record systems to ensure efficient workflow and patient engagement.
