• Conduct thorough assessments and psychotherapy sessions tailored to patients dealing with diverse issues related to grief, anxiety, and depression in a hospice context.
• Utilize a variety of psychological assessments to accurately evaluate clients' mental health requirements and develop personalized treatment plans.
• Develop and implement measurable treatment goals while working closely with patients and their families to address their unique psychosocial needs.
• Provide crisis intervention services, including conducting suicide risk assessments, managing emotional crises, and facilitating hospitalizations when necessary.
• Collaborate with interdisciplinary healthcare teams to ensure coordinated care, enhancing communication among doctors, nurses, and other professionals involved in patient treatment.
• Maintain meticulous documentation of patient interactions, treatment plans, and outcomes, in compliance with legal and ethical standards.
• Assist in shaping clinical policies and procedures that enhance service delivery and uphold the highest standards of care.

As a Clinical Social Worker at our hospice center, you will primarily work with patients facing serious health challenges, including terminal diagnoses, chronic illnesses, and significant life transitions. This role involves providing mental health support to not only the patients but also their families, who often experience stress, grief, and emotional turmoil during these critical times.

• Master's degree in Social Work from an accredited institution and a current LCSW license.
• A minimum of two years of clinical experience in providing mental health services, particularly in a settings involving grief or trauma.
• Strong knowledge of evidence-based treatment modalities such as cognitive-behavioral therapy (CBT), mindfulness-based interventions, and family systems therapy.
• Proven ability to engage effectively with diverse populations and address complex psychosocial issues with sensitivity and care.
• Excellent communication skills, both written and verbal, allowing for effective interaction with patients, families, and healthcare professionals.

• Experience providing care within hospice or palliative care settings.
• Familiarity with electronic health record (EHR) systems and documentation requirements in a clinical environment.
• Prior experience in crisis intervention and management.
