• Conduct thorough clinical assessments of patients and their families, developing personalized care plans that address individual needs and preferences while ensuring compliance with hospice care standards.
• Implement therapeutic interventions using evidence-based modalities, including cognitive-behavioral therapy and solution-focused techniques, to provide emotional support and coping strategies during end-of-life transitions.
• Maintain accurate, confidential clinical documentation in adherence to HIPAA regulations and organizational policies, ensuring continuity of care and effective communication within the treatment team.
• Collaborate with a dedicated team of medical professionals, including physicians, nurses, and chaplains, to provide comprehensive support that respects the dignity and wishes of each patient.
• Facilitate family meetings to educate and guide family members through the complexities of hospice care and bereavement processes, empowering them to support their loved ones effectively.
• Participate in regular team meetings and case consultations, contributing your insights and suggestions to ensure the highest standards of patient care are maintained.
• Stay updated on developments in palliative care and social work practices, bringing the most current knowledge to your practice to enhance patient outcomes.

As an LCSW in our Hospice Center, you will be predominantly working with terminally ill patients and their families during what can be a very challenging period. Your role will involve addressing emotional distress, facilitating effective communication, and providing supportive resources both during and after the hospice experience. Your expertise in trauma-informed care will enhance the support offered to individuals facing the complexities of grief, loss, and transition.

• A Bachelor's degree in Social Work from an accredited institution is required, along with an active LCSW licensure in the state of Florida.
• Proficient in developing individualized treatment plans that reflect a deep understanding of patients' unique circumstances and needs.
• Strong communication and interpersonal skills are essential for building meaningful relationships with clients and their families.
• Ability to use critical thinking and problem-solving skills to navigate complex situations and provide emotional support effectively.
• Demonstrated capacity to work collaboratively within a multidisciplinary team focused on delivering patient-centered care.

• Experience in hospice, palliative care, or mental health settings is highly desirable and will enrich the care we provide.
• Familiarity with electronic health record (EHR) systems to ensure accurate documentation and communication across the care team.
• Previous work involving trauma-informed practices or grief counseling will strengthen your capability to support our diverse patient population.
