As a Licensed Clinical Social Worker, you will perform a variety of essential tasks aimed at helping patients navigate their mental health needs during end-of-life care. Your specific responsibilities include:

• Conducting thorough clinical assessments to understand patients’ and families’ emotional and psychological needs.
• Developing personalized treatment plans that align with established best practices and solution-based interventions.
• Administering diverse therapeutic methodologies, such as cognitive-behavioral therapy (CBT) and mindfulness techniques, customized to meet the unique needs of patients and their families.
• Maintaining accurate and compliant clinical documentation in line with HIPAA regulations, ensuring both privacy and security in patient care.
• Collaborating with an interdisciplinary team of healthcare providers, including physicians and nurses, to coordinate holistic and effective patient care.
• Leading case consultations and participating in peer review sessions to continuously improve care standards.
• Providing educational resources and emotional support to patients and family members regarding mental health issues and available community resources, helping them feel informed and empowered during difficult times.
• Staying abreast of the latest research developments in hospice care and integrating new findings into your clinical practice to enhance treatment efficacy.

Your primary focus will be on patients in hospice care, many of whom face complex emotional issues surrounding grief, loss, and terminal illness. You will work closely with patients and their families, providing critical support that fosters a meaningful and peaceful transition during end-of-life care. Recognizing the unique challenges faced in this sensitive setting will guide your therapeutic approach.

To be successful in this role, candidates must possess the following qualifications:

• A Bachelor’s Degree in Social Work from an accredited institution; a Master’s degree is preferred.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in Florida.
• A strong understanding of evidence-based psychotherapy practices, particularly in the context of hospice or palliative care.
• Exceptional communication and active listening skills, complemented by the ability to establish rapport with patients and families.
• Demonstrated capacity for critical thinking and evidence-based decision-making in clinical practice.
• A collaborative spirit, with the ability to work effectively within a multidisciplinary team.

While not mandatory, the following qualifications are preferred:

• A minimum of three years of experience in a clinical setting, with specialization in hospice, palliative care, or end-of-life counseling.
• Proficiency in utilizing electronic health records (EHR) for clinical documentation and patient management.
