This position is fully remote, allowing you the flexibility to perform your duties from the comfort of your home while still being an integral part of our dedicated hospice care team. You will collaborate closely with physicians, nurses, and other mental health professionals through our innovative telehealth platforms, which enhance communication and coordination of care for our patients and their families.

• Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, and environmental needs, formulating individualized care plans that address identified challenges.
• Provide evidence-based therapeutic strategies, including cognitive-behavioral therapy and solution-focused brief therapy, specifically tailored to the unique concerns of terminally ill patients.
• Work alongside interdisciplinary team members—doctors, nurses, and therapists—to ensure a holistic and coordinated approach to patient care, optimizing clinical outcomes.
• Assist patients and their families in understanding the complexities of end-of-life care, including treatment options, advance care planning, and grief counseling.
• Facilitate individual, group, and family therapy sessions aimed at addressing emotional distress, coping strategies, and enhancing overall quality of life during hospice care.
• Advocate for patients' rights and needs within the healthcare system, helping navigate resources and support services to enhance their experience and care.
• Develop and maintain therapeutic relationships with patients and families, offering guidance and support throughout their hospice journey.

In this role, you will primarily focus on individuals in hospice and palliative care settings, assisting patients grappling with terminal illnesses and their families. Your support will be critical in helping patients find meaning and peace during their final stages of life while equipping families with the tools to navigate grief and loss.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of social work practices and mental health disorders, particularly related to end-of-life care.
• Demonstrated ability to work collaboratively in a multidisciplinary team environment, emphasizing teamwork and effective communication.

• At least two years of experience in hospice, palliative care, or a mental health setting.
• Knowledge of advanced care planning processes, grief counseling techniques, and psychosocial support strategies.
• Experience with telehealth technologies and remote therapy environments.

This full-time role offers a competitive salary ranging from $80,000 to $100,000 annually, commensurate with experience and qualifications. In addition to salary, we provide:
