• Conduct comprehensive assessments to evaluate the emotional and psychological states of patients, considering their unique backgrounds and presenting issues.

• Develop personalized treatment plans in collaboration with patients and their families, regularly assessing progress and making necessary adjustments to interventions.
• Engage actively in team discussions to coordinate integrated care strategies, ensuring alignment across different facets of the patient’s care including physical and emotional aspects.
• Maintain stringent confidentiality and adhere to ethical guidelines, legal requirements, and best practices in the field of social work.
• Participate in continuous professional development to stay current with evolving treatments and methodologies, thereby enhancing the quality of care provided.
• Foster trust and rapport with patients and their families, serving as a source of support while guiding them through complex emotional terrain associated with end-of-life issues.
• Provide practical strategies to families for coping with grief and loss, offering resources and referral information as needed.

The ideal LCSW candidate will work with a diverse range of patients in a hospice setting, primarily focusing on individuals coping with terminal illnesses, as well as their families. The social worker will play a critical role in navigating difficult conversations, addressing anticipatory grief, and offering ongoing support to ensure the mental well-being of both patients and their loved ones.

• A Master's degree in Social Work (MSW) from an accredited institution is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
• A minimum of two years of clinical experience, ideally within a hospice or palliative care context, is preferred.
• Strong clinical assessment, treatment planning, and intervention skills are mandatory.
• Exceptional communication, emotional intelligence, and interpersonal abilities necessary for effectively connecting with patients and families.
