Registered Nurse (RN) Case Manager – Hospice | West Valley, AZ
Care Plus Hospice is seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to join our growing hospice team serving the West Valley. Position Summary
The RN Case Manager is responsible for coordinating and managing hospice care for an assigned caseload of patients. This role works closely with the interdisciplinary team to develop individualized plans of care, provide skilled nursing services, educate patients and families, and ensure quality care throughout the hospice journey.
Responsibilities
  •  Manage and coordinate an assigned hospice patient caseload. 
  •  Perform comprehensive nursing assessments. 
  •  Develop, implement, and update individualized plans of care. 
  •  Provide skilled nursing visits in patients' homes, assisted living facilities, and skilled nursing facilities. 
  •  Collaborate with physicians, nurse practitioners, social workers, chaplains, CNAs, and other interdisciplinary team members. 
  •  Educate patients and families regarding disease progression, symptom management, medications, and hospice services. 
  •  Participate in interdisciplinary team (IDT) meetings. 
  •  Complete accurate and timely electronic documentation. 
  •  Participate in on-call rotation as assigned. 
  •  Ensure compliance with Medicare Conditions of Participation and hospice regulations. 
Qualifications
  •  Current Arizona Registered Nurse (RN) license in good standing. 
  •  Minimum one year of nursing experience required. 
  •  Hospice or home health experience preferred. 
  •  Strong assessment, communication, and critical thinking skills. 
  •  Ability to work independently while collaborating with an interdisciplinary team. 
  •  Current CPR certification. 
  •  Valid Arizona driver's license, reliable transportation, and current auto insurance.
