This role is fully remote, allowing for flexibility while still providing essential support to our hospice center. As an LCSW, you will connect with colleagues via virtual meetings and utilize advanced technologies to facilitate assessments and therapy sessions. • Conduct comprehensive evaluations and assessments of patients’ mental health needs and psychosocial factors affecting their care.
• Implement behavior management strategies to monitor and manage patient behaviors effectively, ensuring a safe and therapeutic environment.
• Work collaboratively with a multidisciplinary team including physicians, nurses, and other healthcare professionals to develop and deliver integrated treatment plans.
• Provide individual and group counseling or psychotherapy utilizing evidence-based practices aligned with the latest advancements in third-wave therapies.
• Monitor, document, and communicate patient progress, adjusting care plans as needed to enhance treatment outcomes.
• Engage with families by providing vital information, support, and skills training related to their loved one's development and care.
• Maintain meticulous documentation of clinical interactions and treatment plans in our electronic health record system, adhering to compliance standards and timelines.

Our patient demographic includes individuals facing terminal illnesses and their families, requiring a specialized understanding of not only grief and loss but also the complex emotional and psychological struggles associated with end-of-life care. As an LCSW at Sunny Skies, you will navigate these sensitive issues, ensuring that patients receive both emotional support and practical coping strategies.

• A Master's degree in Social Work, Counseling, or a related field is required.
• Must possess a valid LCSW license in the state of California.
• A minimum of 3 years of experience in providing clinical services to diverse populations, particularly in a hospice or palliative care setting, is essential.
• Proficiency in computer operations, including MS Outlook, Word, and relevant electronic health record systems.
• Demonstrated ability to complete clinical documentation accurately and in a timely manner.

While the core qualifications ensure a qualified candidate, experience with third-wave therapeutic approaches, such as Acceptance and Commitment Therapy (ACT) or Dialectical Behavior Therapy (DBT), is highly preferred. Prior work in hospice or palliative care settings will be invaluable in effectively addressing the unique challenges faced by our patients and their families.
