• Conduct comprehensive psychosocial assessments to understand the emotional, social, and spiritual needs of patients and their families.
• Develop and implement individualized treatment plans aimed at addressing complex issues such as grief, anxiety, and depression related to terminal illness.
• Facilitate individual and group therapy sessions to empower patients and their families through their emotional journey.
• Provide direct support and guidance in crisis situations, helping patients and families navigate decisions about care and coping strategies.
• Document all interactions and interventions accurately and confidentially in compliance with regulatory standards and organizational policies.
• Collaborate regularly with other healthcare professionals to ensure integrated care and effective communication regarding patient progress.
• Maintain an up-to-date knowledge of best practices, therapeutic modalities, and legislation relevant to social work and hospice care.

As a Licensed Clinical Social Worker in our hospice center, you will work primarily with patients facing life-limiting illnesses and their families. Your role will involve addressing a broad spectrum of emotional challenges, including anticipatory grief, loss, and the psychological burdens often encountered in hospice scenarios. This position demands a deep understanding of the unique dynamics of terminal illness and the importance of dignified end-of-life care.

• Master's Degree in Social Work from an accredited institution.
• Valid and active LCSW license in the state of New Jersey, demonstrating your expertise in clinical social work.
• Exceptional verbal and written communication skills, essential for building rapport with patients and collaborating with multidisciplinary teams.
• A strong commitment to patient advocacy and ethical practice, emphasizing the dignity of those receiving hospice care.

While a minimum of two years’ experience in a clinical or hospice setting is required, preference will be given to candidates with a background specifically in end-of-life care or a demonstrated ability to work with seriously ill patients.
