• Conduct thorough clinical assessments to evaluate patient needs and develop personalized treatment plans tailored to individual circumstances.
• Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy and solution-focused brief therapy, to assist patients in managing their mental health challenges.
• Maintain meticulous and timely clinical documentation that adheres to best practice guidelines, ensuring compliance with regulatory requirements.
• Collaborate closely with a multidisciplinary treatment team, including physicians, nurses, and counselors, to coordinate care and share insights on patient progress.
• Educate and support patients and their families through care transitions, providing resources and guidance to facilitate understanding and acceptance of hospice care.
• Stay informed on the latest research and developments in the field of mental health to continually enhance both your practice and the outcomes for your patients.

You will work with a diverse range of patients and families located in Independence, Missouri, providing clinical support during some of the most challenging times of their lives. Our patients often face significant emotional and psychological distress, and your role will involve not only treating those issues but also offering comfort and guidance as they navigate the complexities of hospice care.

• A Master's degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Missouri.
• Proven experience with solution-focused or problem-solving psychotherapy approaches.
• Exceptional communication and interpersonal skills, with an ability to build rapport and trust with patients and families quickly.
• Strong organizational skills and attention to detail, particularly in maintaining clinical documentation.

• Experience working in hospice or palliative care settings, with an understanding of the unique emotional needs of terminally ill patients.
• Familiarity with telehealth practices and technology, as our work environment relies on remote interaction.
• Previous involvement in interdisciplinary teams to enhance collaborative care.
