LPN/RN MDS Coordinator (Medicare/Medicaid Expert) - St. Louis, Missouri

As an LPN/RN MDS Coordinator, you will play a vital role in ensuring the delivery of exceptional care to our residents, while also ensuring compliance with Medicare and Medicaid regulations.

Job Summary:

As an LPN/RN MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintaining compliance with Medicare and Medicaid regulations. You will work closely with our care team to develop and implement individualized care plans that meet the unique needs of our residents.

Responsibilities:

  • Coordinate the MDS assessment process, including scheduling and conducting assessments, and reviewing assessments for accuracy and completeness

  • Ensure timely completion of MDS assessments and communicate with families and healthcare providers as necessary

  • Develop and implement individualized care plans that meet the unique needs of our residents

  • Collaborate with the care team to identify and address resident needs and concerns

  • Maintain accurate and up-to-date records and documentation

  • Participate in quality improvement initiatives to ensure compliance with Medicare and Medicaid regulations

Requirements:

  • Current LPN or RN licensure in the state of Missouri
