• Conduct thorough assessments of patients to understand their emotional and social needs and to formulate comprehensive, individualized treatment plans.
• Implement evidence-based therapeutic interventions aimed at improving patient outcomes and quality of life, focusing on anxiety, depression, and trauma.
• Maintain detailed and confidential records of patient progress and treatment, employing our electronic health record (EHR) system for documentation.
• Work collaboratively with a multidisciplinary team, including physicians, nurses, and other health professionals, to ensure holistic care for patients.
• Regularly engage in professional development to stay current with evolving best practices and research in social work and hospice care.
• Provide emotional support and counseling to family members facing the challenges of a loved one’s illness and end-of-life decisions.

As a Licensed Clinical Social Worker specializing in hospice care, you will serve a diverse population of individuals and families navigating the complexities of terminal illness, grief, and end-of-life decisions. Our clients often face emotional distress, requiring sensitive and tailored therapeutic strategies to support their unique situations. You will play a key role in empowering patients and their families, helping them cope with the emotional demands of serious health challenges.

• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• Active LCSW (Licensed Clinical Social Worker) license in the state of Connecticut.
• Exceptional communication skills, with a focus on empathetic listening and problem-solving.
• Ability to thrive in a collaborative, multidisciplinary team environment.
• Strong organizational skills and attention to detail when managing documentation and patient records.

• Experience in hospice, palliative care, or similar settings is highly desirable.
