• Conduct comprehensive clinical assessments to identify patients’ psychological and emotional needs related to hospice care.
• Develop and implement individualized treatment plans that align with patient goals and values, regularly reviewing and adjusting these plans as necessary.
• Administer evidence-based therapeutic interventions, including individual and family counseling sessions, ensuring that these practices are tailored to the unique challenges faced by patients and their loved ones.
• Collaborate with a multidisciplinary treatment team, including nurses, physicians, and spiritual counselors, to ensure a holistic approach to patient care.
• Provide education and emotional support to both patients and their families, equipping them with coping strategies and resources for end-of-life decisions.
• Maintain accurate and timely clinical records in compliance with state regulations and organizational policies, ensuring that documentation reflects the care provided.
• Stay informed on the latest research and developments in the field of social work and hospice care, integrating new knowledge into practice.

In this role, you will work primarily with patients facing terminal illnesses and their families, who may be experiencing a wide range of emotions, from grief and loss to anxiety and fear about the dying process. You will provide essential support for patients' mental health needs during this critical time, facilitating open communication and understanding within families dealing with difficult emotions and decisions.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Connecticut.
• Demonstrated knowledge of solution-focused and evidence-based psychotherapy interventions.
• Exceptional communication and problem-solving skills, with the ability to empathize and build rapport with a diverse clientele.

• Experience in hospice or palliative care settings, focusing on end-of-life counseling and support.
• Familiarity with the psychological aspects of chronic illnesses and end-of-life issues.
• Prior involvement in multidisciplinary care teams, demonstrating collaborative approaches to patient care.
