This role is primarily remote, allowing for flexibility in your schedule while connecting with clients directly in their homes within the community. The work hours are Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends required based on client needs. You will have the autonomy to manage your schedule, providing on-call availability as needed to address urgent situations.

As a Home Care Social Worker, your responsibilities will include:

• Conducting comprehensive assessments of clients' psychosocial needs, which involve evaluating emotional, mental, and social factors affecting their well-being.
• Developing individualized care plans in close collaboration with clients and their families, setting specific, achievable goals tailored to their preferences and situations.
• Providing high-quality counseling and emotional support to seniors and their families, helping them manage various transitions and overcome challenges encountered during the aging process.
• Advocating for clients’ rights and providing guidance on accessing community resources for additional support and services.
• Collaborating with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate comprehensive care and enhance client outcomes.
• Conducting regular home visits to monitor clients’ well-being, ensuring that their living conditions are safe and supportive.
• Facilitating effective communication among family members involved in the care process, working to resolve conflicts and promote understanding.
• Teaching clients and their families about available community resources, support groups, and social programs that can enrich their lives.
• Maintaining precise and timely documentation of client assessments, care plans, and progress notes, adhering to all regulatory standards and ethical practices.
• Staying updated on best practices in social work and home care, along with relevant policies, laws, and ethical regulations.

The primary focus of this role is on senior clients—individuals aged 65 and older—who may be dealing with the complexities of aging, including physical health issues, mental health challenges, and social isolation. You will work closely with clients from diverse backgrounds, understanding their unique life experiences and leveraging that knowledge to develop customized care strategies.

• Bachelor’s Degree in Social Work or a related field from an accredited institution.
• Current and valid state licensure as a Social Worker (LCSW, LMSW, or equivalent preferred).
• Minimum of two years of experience in a social work role, ideally in home care or geriatric settings.
• Strong understanding of social work principles, aging-related issues, and community resources available to seniors.
• Exceptional assessment, counseling, and problem-solving skills.
• Demonstrated empathy and compassion, with strong relationship-building capabilities.
• Excellent communication and interpersonal skills, essential for effectively interacting with clients, families, and other team members.

• Experience working in a home care agency or geriatric healthcare environment.
• Familiarity with healthcare software and documentation practices relevant to social work.
• Proven ability to advocate for patients and navigate complex healthcare and community resources.
