This remote position allows you to work from home while engaging with individuals who are in hospice care across Seattle, Washington. • Conduct comprehensive psychosocial assessments to evaluate patients' mental health status, strengths, and needs. Examples include identifying emotional distress, coping mechanisms, and social support systems.
• Implement evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) tailored to each patient's unique situation.
• Collaborate effectively with psychiatrists, nurses, and other disciplines to coordinate holistic care plans that address both physical and emotional needs.
• Educate patients and their families about diagnoses, treatment options, and medication management to empower informed decision-making.
• Facilitate individual, group, and family therapy sessions, utilizing modalities that best fit the needs of participants.
• Develop and maintain treatment plans in collaboration with the healthcare team, ensuring patient goals and preferences are central to the process.
• Assist patients in navigating the mental health system, advocating for their rights and needs throughout their treatment journey.
• Lead therapeutic groups and psychoeducation sessions, engaging patients in meaningful discussions and discharge planning meetings.

Your primary focus will be on patients receiving hospice care who may be experiencing a wide range of mental health challenges, including grief, anxiety, depression, and existential distress. By understanding the unique needs of individuals in this sensitive phase of life, you will provide tailored interventions that contribute to their emotional and mental well-being. • A Master's degree in Social Work from an accredited program is essential.
• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is required to practice.
