Provides care ordered by the physician and directed by the registered nurse or appropriate skilled professional as indicated by the plan of care to all patients on caseload including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding, and transferring patient.

Takes accurate vital signs.

Demonstrates competency in required skills per Competency Skills Checklist.

Describes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activities.

Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.

Notifies appropriate licensed staff when patient complains of pain.

Recognizes and reports changes in skin condition.

Positions patient in bed with proper body alignment.

Assists patient with ambulation and transfer techniques as necessary.

Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

Supports cultural practices as long as these practices do not harm the patient/family or interfere with the planned course of medical treatment.

Identifies and communicates findings to the Case Manager and/or Clinical Manager.

Participates in HHA's other mandated activities, as requested by Clinical Manager or Director of Patient Care Services (DPCS).

Provides clear, concise documentation in medical record as it relates to reimbursement guidelines and Agency policy and procedure.

Home Health Aide certificate obtained by:

  • Completion of a certified home health aide course.
  • Certified by State Department of Health Services.

Successful completion of a State-established or other training program that meets CMS requirements (484.36(a)) and a competency evaluation program or State licensure program that meets CMS requirements (484.36(b) or (e)).

Note: A Home Health Aide or nurse aide is not considered to have completed a program, as specified in 42 CFR 484.80(a)(1), if, since the individual's most recent completion of the program(s), there has been a continuous period of 24 consecutive months during which none of the services furnished by the individual as described in 42 CFR 409.40 were for compensation. If there has been a 24-month lapse in furnishing services for compensation, the individual must complete another program, as specified in 42 CFR 484.80(a)(1), before providing services.

On-the-job instruction relative to specific patient needs and continuing inservice training, so that patient safety may be assured and the personal care and homemaking needs of the patient are adequately met.

Prior home health experience preferred.

Valid driver's license and reliable transportation

Proof of automobile insurance (MVR checked)

Proof of current CPR certification

Able to pass criminal background check and national sex offender clearance
