Key Responsibilities

• Evaluate, diagnose, and treat a variety of psychosocial issues that patients encounter, using clinical social work skills to facilitate their healing process.
• Conduct comprehensive bio-psychosocial evaluations to identify barriers to treatment and develop personalized care plans aimed at enhancing patient well-being.
• Collaborate with an interdisciplinary team, exchanging detailed patient histories and insights to create integrated treatment strategies, thereby improving care coordination and outcomes.
• Utilize evidence-based therapeutic modalities in both individual and group settings, analyzing treatment efficacy to optimize health improvement.
• Document all social work activities comprehensively in patients’ medical records, adhering to departmental standards and regulatory requirements.
• Provide family education on defining and following medical and therapeutic recommendations, empowering them to support the patient’s recovery journey.
• Maintain knowledge of community resources and advocate for patients, ensuring they access needed support services to enhance their overall quality of life.
• Participate in ongoing professional development and training, keeping current with best practices in social work and mental health.

Patient Population / Client Focus

This role focuses particularly on supporting individuals in hospice care, often managing complex emotional and psychological challenges due to terminal illnesses. Understanding the intricacies of aging, human developmental stages, and cultural factors is essential as you work with a diverse clientele, ensuring that care is both culturally competent and patient-centered.

Qualifications

• A Master’s Degree in Social Work from an accredited program and a current state license as a Licensed Clinical Social Worker (LCSW) in Colorado.
• Proven knowledge and skills in social work practice that consider cultural sensitivities and the unique needs of each patient.
• Strong clinical skills, analytical abilities, and an understanding of best practices in mental health treatment.

Preferred Experience

• A minimum of one year of direct clinical experience, ideally within health or mental health settings that service similar populations.
• Previous experience providing care in hospice or palliative settings, including familiarity with grief counseling and loss.
