Key Responsibilities
  • Plan and implement personalized support packages that address the unique needs of clients, helping them navigate crises and overcome dependencies.
  • Conduct thorough assessments through interviews to evaluate clients' current conditions, strengths, and areas for improvement, tailoring your approach based on individual circumstances.
  • Establish and monitor measurable goals for each case, regularly updating treatment plans and strategies based on clients' progress.
  • Provide crisis intervention during sessions, utilizing effective communication techniques to de-escalate situations and guide clients toward appropriate solutions.
  • Maintain accurate documentation of client interactions, assessments, and progress to ensure compliance and continuity of care.
  • Attend case conferences and collaborate effectively with other professionals, providing expert testimony as needed in legal proceedings.
  • Actively refer clients to community resources (such as support groups and rehabilitation services) to facilitate their recovery and reintegration.

Qualifications
