Primary Care Physician (Remote | Full-Time | No On-Call) Monday- Friday 9am-6pm

What You'll Do

  • Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
  • Create and manage personalized care plans for ongoing conditions

Requirements

What We're Looking For

  • MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
  • Active medical license - multi-state licensure a plus
  • Credentialed with major plans: UHC, Aetna, Humana
  • Strong interest in preventive care, senior health, or chronic condition management
  • Tech-savvy and comfortable practicing via telehealth
  • Collaborative, mission-driven mindset with excellent communication skills
