• Conduct comprehensive clinical assessments using telehealth platforms, thoroughly evaluating patients' mental health needs in the context of hospice care.
• Develop and implement individualized treatment plans based on established best practices, employing evidence-based therapeutic techniques such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).
• Maintain accurate clinical records and ensure compliance with legal and ethical standards, utilizing secure electronic health record (EHR) systems for documentation and patient management.
• Collaborate closely with a diverse treatment team, including psychiatrists, psychologists, and nursing staff, through virtual meetings to discuss patient progress and devise optimal care strategies.
• Provide educational resources and counseling to patients and their families, supporting them in navigating mental health challenges and accessing relevant community resources.
• Participate in regular case consultations and peer reviews, contributing to a culture of continuous improvement and enhanced patient care through feedback and discussion.

You will work with a diverse range of patients, primarily individuals in hospice care dealing with end-of-life concerns. Your focus will be on providing emotional support, addressing anxiety and grief, guiding families through the complex emotions associated with terminal illness, and helping patients find dignity in their final days.

• A Master's degree in Social Work from an accredited institution.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• A minimum of 2 years of direct clinical experience, particularly in providing psychotherapy services within hospice or palliative care settings.
• Demonstrated ability to employ solution-focused interventions suitable for remote delivery.
• Exceptional communication and interpersonal skills, with the capacity to build rapport and trust with patients and their families through virtual platforms.
• Strong analytical and problem-solving skills, effectively navigating challenges associated with remote healthcare delivery.
