• Conduct comprehensive patient intakes and assessments, focusing on detailed diagnostic and functional evaluations. Use these evaluations to create personalized treatment plans in collaboration with primary care providers.
• Provide targeted therapeutic interventions designed to improve the emotional and psychological well-being of patients, employing positive role modeling and evidence-based practices.
• Monitor ongoing patient progress, adjusting treatment plans as necessary based on patient feedback and clinical observations.
• Deliver educational resources and self-management skills to empower patients in their own treatment journey, fostering independence in managing their mental health.
• Work collaboratively with primary care providers to recognize and manage various mental health and psychosocial challenges, ensuring optimal patient outcomes.
• Develop and implement relapse prevention plans for patients, identifying risk factors and creating strategies to mitigate them.
• Coordinate with external treatment providers, enhancing the continuum of care and ensuring patients receive comprehensive support.
• Remain fully informed about the patients' needs and progress, maintaining accurate and up-to-date clinical documentation within treatment charts.
• Participate in team meetings and contribute to program evaluation discussions, providing insights into service delivery and effectiveness.
• Adhere strictly to the professional ethics code to maintain the trust and confidentiality of all consultations.

• Thorough knowledge of developmental psychopathology and systems approaches in family and group therapy.

• Experience working within Federally Qualified Health Centers (FQHCs) or similar community health settings is an advantage.
• Bi-lingual proficiency in English and Spanish is highly desirable, enabling us to serve a broader client population.
