Key Responsibilities
  • Conduct comprehensive assessments and create individualized treatment plans tailored to meet the unique needs of each client.
  • Complete all necessary documentation promptly and accurately, adhering to regulatory and organizational standards, to ensure compliance and quality care.
  • Utilize evidence-based screening tools in collaboration with the clinical team to monitor client progress and adapt treatment plans as necessary.
  • Develop and implement positive behavior support plans in cooperation with multidisciplinary teams, ensuring that all interventions are well-coordinated.
  • Actively engage in licensing and accreditation processes, ensuring that all services meet the required quality and ethical standards.
  • Serve as a liaison to relevant service systems, state agencies, and community partners to ensure comprehensive support for clients and facilitate their access to resources.
  • Maintain open communication channels within the team and the community to uphold our mission and service contracts effectively.
  • Establish and strengthen partnerships with local, state, federal, and private organizations to promote our programs and identify new opportunities for collaboration and funding.
