• Conduct comprehensive assessments of patients through bio-psychosocial evaluations to identify barriers to treatment discharge and optimize health outcomes.
• Develop and implement individualized treatment plans utilizing evidence-based modalities, leveraging individual and group therapy techniques to enhance patient care.
• Closely collaborate with a multidisciplinary team, including medical and support staff, to provide integrated care and improve treatment compliance.
• Document social work activities meticulously in patients' medical records, ensuring compliance with departmental standards and regulations.
• Advocate for patients and their families by connecting them to community resources and education that facilitate recovery and self-determination.
• Maintain current knowledge of available community resources and client advocacy needs to effectively address psychosocial challenges.
• Participate in continuous professional development by attending relevant training and educational programs.

The patient population you will serve includes individuals facing a variety of psychosocial challenges related to their mental health and wellbeing. This includes working with children, adolescents, and adults dealing with the complexities of mental illnesses and substance use disorders. Your role will be essential in providing empathetic support to both patients and their families, bridging gaps in healthcare access.

• A Master’s Degree in Social Work from an accredited program is required.
• A current state license in Social Work at the Licensed Clinical Social Worker (LCSW) level.
• Demonstrated proficiency in applying clinical skills to address varying ages and cultural patterns in treatment.

• At least 1-2 years of clinical experience within health or mental health environments is preferred.
• Experience providing therapy to diverse populations, particularly with an emphasis on age-related developmental stages.
• Prior exposure to outpatient settings where telehealth technologies are utilized.
