• Conduct clinical evaluations and develop individualized aftercare plans that facilitate successful client reintegration after discharge.
• Monitor and document medication effects in collaboration with psychiatrists to ensure comprehensive client management.
• Facilitate ongoing case management through regular consultation, treatment team meetings, and utilization reviews to optimize care pathways.
• Ensure meticulous documentation of all client interactions, including biopsychosocial assessments, progress notes, therapeutic goals, and discharge planning.
• Actively research and connect clients with community resources such as food assistance, childcare services, and healthcare facilities to improve overall well-being and social stability.
• Engage in crisis intervention as needed, employing strong problem-solving skills to address emergency situations effectively.
• Participate in training and professional development opportunities to enhance service delivery and remain informed on best practices in social work.

As a Senior LCSW, you will be tasked with understanding the diverse backgrounds and cultural needs of our clients, ensuring you provide tailored support that resonates with their personal experiences. • Master's Degree in Social Work from an accredited university.
• Active and unrestricted California Clinical Social Worker License (LCSW).
• A minimum of two years of professional experience in a behavioral health setting, demonstrating skill in case management and therapeutic practice.
• Proven competency in conducting psychosocial evaluations, including knowledge of interview techniques and risk assessments for suicidality and forensic considerations.
• Strong conflict resolution skills and the ability to work effectively under pressure.
• Familiarity with electronic health records systems and telehealth applications.

• Experience in community outreach or coordinating with social services.
• Background in crisis intervention or intensive case management.
• Training in culturally competent care and trauma-informed practices.
