Summary
About the Position: This position is located at Naval Medical Center San Diego with Directorate of Mental Health.

Duties
Help
  • Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient.
  • Track client progress towards goals, modifies care plans as needed, and advocates for clients' rights and preferences.
  • Observe and evaluate patient behavior and responses, updating treatment plans in consultation with the healthcare team.
  • Monitor psychotropic medication use, tracks physical and emotional side effects, recommends adjustments to therapy modalities, appropriate level of care placement, and provide clinical feedback to providers.
  • Work with mental health clinic staff to ensure patient needs are met based upon the patient's assessed condition.
  • Collect and analyze data to identify trends and patterns and makes recommendations for program enhancements.
  • Coordinate medical services needed to optimize current health, psycho-social, spiritual status, and benefits.
  • Conduct training, education, counseling, and clinical assistance in mental health case management techniques to nursing personnel, interns/residents, and administrators.

  • Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
  • This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination.
  • Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series.
  • Appointment to this position is subject to a one year probationary period unless the appointee has previously met the requirements as described in 5 CFR Part 315. While probationary the incumbent can be terminated with limited appeal rights.
  • This position is a Testing Designated Position subject to pre-employment screening and random drug testing. Selectees will be required to consent to participation in random drug urinalysis testing.
  • Position may be designated as Mission Essential. The incumbent will be required to report to work during times of contingency operations, natural disasters, extreme weather or other emergency situations.
  • Must be able to obtain and maintain current Basic Life Support (BLS) Training and certification sponsored or endorsed by the American Heart Association (AHA). Advanced certification (e.g., Advanced Life Support) does not supersede BLS completion.
  • A personnel Tier 3 secret security investigation is required.

In order to qualify, you must meet the education and experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Your resume must clearly describe your relevant experience; if qualifying based on education, your transcripts will be required as part of your application. Basic Requirement for Mental Health Case Management:
Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences, or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Specialized Experience: One year of specialized experience which includes providing mental health case management services such as conducting patient assessments or monitoring patient progress; coordinating with medical providers to ensure continuity of care for patients; and participating as a member of a multidisciplinary treatment team reviewing patient cases. This definition of specialized experience is typical of work performed at the second lower grade/level position in the federal service (GS-11).
Some federal jobs allow you to substitute your education for the required experience to qualify. For this job, you must meet the qualification requirement using experience aloneno substitution of education for experience is permitted.
