Key Responsibilities

• Act as a liaison between clients, families, and outpatient providers to optimize care coordination and ensure seamless communication.
• Work with insurance companies to facilitate pre-certification and manage utilization effectively, ensuring clients receive necessary services.
• Engage in client discharge planning and aftercare coordination to foster long-term health and continued support.
• Provide therapeutic meal support by modeling positive relationships with food, guiding clients in adopting healthier eating habits.
• Deliver empathetic, culturally competent care tailored to each client's intersecting identities, ensuring a safe space for healing and empowerment.

Patient Population / Client Focus

In your role, you will engage with a diverse patient population that includes children, adults, and older adults experiencing various mental health challenges. We cater to clients needing depression and anxiety management, trauma recovery, and other behavioral health issues. Your skills will be pivotal in fostering trust and rapport with clients from different backgrounds, helping to break down barriers to accessing mental health services.

Qualifications

• Master's degree in Social Work from an accredited institution is required, showcasing your expertise and commitment to the field.

Compensation and Benefits

This full-time position offers a competitive salary ranging from $90,000 to $105,000 annually, commensurate with experience and education.
