Key Responsibilities

• Conduct comprehensive nutrition assessments, which include evaluating patients’ dietary habits, medical history, and lifestyle factors to develop customized nutrition care plans.
• Provide in-depth nutrition counseling and education, using evidence-based resources to promote healthy eating habits, disease management, and preventive strategies.
• Monitor patient progress by assessing the impact of nutritional interventions, modifying care plans when necessary, and ensuring continuity of care through collaboration with physicians and other healthcare providers.
• Perform nutrition-focused physical examinations and analyze biochemical data to gain insights into patients’ nutritional statuses and needs.
• Engage in regular interdisciplinary team meetings, sharing insights and collaborating on comprehensive treatment plans that address not only physical health but also psychological and lifestyle factors affecting patients.
• Provide guidance to patients with varied dietary restrictions and needs, such as tailored plans for diabetes management, food allergies, or gastrointestinal disorders.
• Maintain accurate patient records in compliance with healthcare regulations and best practices, documenting interactions, assessments, and outcomes effectively.
• Stay abreast of the latest developments in nutrition science and clinical practices to continually enhance care and ensure patients receive the best possible services.

Patient Population / Client Focus

This position focuses primarily on serving patients with diabetes and related health conditions. You will work closely with individuals across various demographics, providing sensitive and thoughtful care that takes into account their unique medical histories, cultural backgrounds, and personal health goals. Your role will directly influence their ability to manage their diabetes effectively and improve overall health outcomes.

Qualifications

• Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited institution.
• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification from the Commission on Dietetic Registration (CDR).
• Active state licensure or eligibility to practice as a Dietitian/Nutritionist in California.
• Strong foundational knowledge of nutrition science, dietary guidelines, and medical nutrition therapy principles applicable to diabetes management.
• Excellent communication skills, both verbal and written, essential for effective patient interactions and interdisciplinary collaboration.

Preferred Experience

• Experience in diabetes education or nutrition counseling.
• Familiarity with telehealth platforms or remote patient care methodologies.
• Experience working in a multidisciplinary healthcare setting.

Compensation and Benefits
