This role is designed for a remote work setting, allowing for flexibility and convenience while serving clients in the community. As a vital member of our community health clinic, you will engage with families and children using virtual platforms and remote engagement tools. • Conduct comprehensive evaluations and assessments of children with developmental disabilities, ensuring a thorough understanding of their unique needs.
• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs), collaborating with multidisciplinary teams to create personalized treatment strategies.
• Monitor and document outcomes, providing feedback and adjustments to treatment plans as necessary to optimize each child's progress.
• Administer evidence-based interventions that empower parents and caregivers, offering them the tools and support to nurture their child's development effectively.
• Build strong rapport with children, their families, and colleagues, cultivating a positive and affirming atmosphere conducive to learning and growth.
• Adhere to all relevant Federal and State regulations, ensuring compliance and the highest standards of practice in all activities.

The LCSW will focus primarily on children from birth to three years old, supporting those who may have physical, cognitive, social/emotional, or communication delays. By working closely with these children and their families, you will contribute to the foundational stages of their development, guiding them toward achieving critical milestones during these formative years.

• Master's Degree or Doctorate in Social Work from an accredited program.
• Current licensure as a Licensed Clinical Social Worker (LCSW) from the Department of Public Health.
• A valid driver's license is required to facilitate home visits and community engagement as needed.
• A strong understanding of Natural Learning Environment Practices, demonstrating a commitment to creating supportive learning settings.

• A minimum of 2 years of direct experience working with children within early intervention settings, showcasing expertise in tailoring support to diverse learning styles and needs.
• Proficiency in trauma-informed care practices and experience navigating community-based resources for families.
