Epic Healthy Planet Analyst:

Are you an Epic Healthy Planet expert looking for an opportunity to make a lasting impact on population health initiatives? We're seeking a highly skilled analyst to help optimize and stabilize a growing Healthy Planet environment while supporting value-based care initiatives that directly improve patient outcomes. MUST be Epic Healthy Planet Certified.

Required Qualifications:

  • Epic Healthy Planet Certification (Required)
  • Epic Compass Rose Certification (Strongly Preferred)
  • Experience supporting population health, care management, and value-based care programs
  • Hands-on Epic build, configuration, testing, and optimization experience
  • Experience with reporting, dashboards, and population health analytics
  • Strong understanding of healthcare workflows and clinical operations

Key Responsibilities:

  • Serve as the primary support resource for Epic Healthy Planet and population health workflows
  • Troubleshoot and resolve application issues, user requests, and help desk tickets
  • Stabilize and optimize an existing Healthy Planet environment by identifying and resolving build and workflow issues
  • Complete system build, configuration, testing, and deployment activities
  • Support value-based care initiatives, including:
    • Care Coordination
    • Chronic Care Management (CCM)
    • Transitional Care Management (TCM)
    • Health Maintenance
  • Develop and maintain dashboards, reporting solutions, and population health analytics
  • Partner with operational and clinical stakeholders to gather requirements and improve workflows
  • Maintain system documentation, design specifications, and technical documentation
  • Participate in training, go-live support, testing, and knowledge transfer activities
  • Collaborate on future population health and payer-related initiatives

Pay and Benefits

The pay range for this position is $50.00 - $60.00/hr.
