Job description

Central Admissions Reviewer – Skilled Nursing Facilities

Remote | Must Reside in the Eastern Time Zone
$80,000–$85,000/year | Full-Time | Monday–Friday

ACC is seeking an experienced Central Admissions Reviewer to support admissions across our skilled nursing facilities in South Carolina and Iowa. This is a remote, clinically focused position responsible for reviewing referrals, evaluating patient needs, verifying insurance requirements, and coordinating admission decisions with facility teams and referral sources.

The ideal candidate will have strong skilled nursing or post-acute clinical experience and be familiar with the South Carolina healthcare and referral market. Experience with the Iowa market is also a plus.

What You'll Do
  • Review hospital records, clinical documentation, and referrals to determine whether a facility can safely and appropriately meet the patient's needs.

  • Evaluate referrals for clinical appropriateness, level of care, and admission eligibility.

  • Verify insurance benefits, eligibility, authorization requirements, and payer criteria.

  • Collaborate with facility clinical teams, admissions coordinators, and leadership on admission decisions.

  • Communicate acceptance, denial, or requests for additional information to hospitals, case managers, discharge planners, and other referral sources.

  • Document referral decisions, clinical rationale, and follow-up activity accurately in the CRM or referral tracking system.

  • Coordinate with facility admissions teams regarding bed availability, timing, and placement.

  • Escalate urgent admissions, complex clinical cases, or issues requiring additional review.

  • Participate in daily admissions meetings and census reviews, providing clinical insight and recommendations.

  • Develop and maintain strong relationships with hospital case managers, discharge planners, physicians, and other referral partners.

  • Ensure timely, professional communication throughout the referral and admissions process.

What We're Looking For
  • Active LPN license in good standing required.

  • At least 3 years of experience in skilled nursing, rehabilitation, hospital, utilization review, or discharge planning.

  • Strong knowledge of post-acute care, including Medicare, Medicaid, and managed care requirements.

  • Strong clinical judgment and ability to evaluate complex patient referrals.

  • Excellent written and verbal communication skills.

  • Experience with PointClickCare or similar EMR systems preferred.

  • Familiarity with referral platforms such as CarePort, naviHealth, or similar systems preferred.

  • Knowledge of the South Carolina skilled nursing/post-acute market strongly preferred.

  • Familiarity with the Iowa market is a plus.

  • Comfortable working independently in a fast-paced, multi-facility environment.

  • Strong organizational skills and ability to manage multiple referrals and priorities throughout the day.

Schedule
  • Full-time, typically Monday–Friday

  • Remote position

  • Must reside in the Eastern Time Zone

  • Primarily supporting facilities and referral partners in South Carolina and Iowa
