Key Responsibilities
  • Complete and maintain crisis alerts to coordinate with county crisis services for participants efficiently.
  • Document transitions between hospital/inpatient services and program transfers while collaborating with hospital staff to ensure safe discharges and continuity of care.
  • Utilize crisis de-escalation techniques effectively to support clients during critical moments, particularly during your role as Clinician of the Day.
  • Conduct and document comprehensive risk assessments and develop safety plans tailored to individual client needs.
  • Ensure timely documentation is completed on the same day for clients at increased risk of requiring crisis services.
  • Fill out critical incident reporting forms within 24 hours when necessary, adhering to organizational standards.
  • Participate in after-hours phone support on an alternating schedule to provide ongoing care coordination.
  • Engage proactively with clients to prevent the escalation of crises, fostering independence and resilience wherever possible.
  • Adhere to additional duties as requested to support the team's goals and facilitate optimal client outcomes.
Patient Population / Client Focus

As a Licensed Mental Health Counselor at Clear Mindcare Counseling Services, you will work primarily with a diverse adult and adolescent client population facing various mental health challenges.
