Key Responsibilities
  • Oversee and document transitions from hospital or inpatient services to outpatient programs, ensuring smooth care coordination with hospital staff for patient discharges.
  • Employ crisis de-escalation strategies, serving as the Clinician of the Day, to support individuals in distress and maintain a safe, therapeutic environment.
  • Conduct thorough risk assessments and develop comprehensive safety plans for clients exhibiting increased risk of crisis interactions.
  • Ensure timely documentation and reporting, completing daily notes for individuals at heightened risk and filing critical incident reports within 24 hours when necessary.
  • Participate in after-hours phone coverage on an alternating schedule to provide ongoing care coordination and support.
  • Work proactively with clients to reduce reliance on crisis services and facilitate access to community resources that enhance overall wellness.
  • Maintain updated crisis alerts to ensure effective collaboration with county crisis services.
  • Engage in additional duties as assigned to contribute to the overall success of the team.
Patient Population / Client Focus

Your clients will represent a diverse range of backgrounds and mental health challenges. You will work with individuals facing complexities such as anxiety, depression, trauma-related disorders, and relational issues. By fostering trusted relationships through empathetic listening and individualized care, you will help clients navigate their unique challenges toward recovery and improved mental health.

Qualifications
  • A Master’s or Doctorate degree in psychology, counseling, or social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
  • Possess an independent state license (LCSW, LMFT, LPC) valid in your residence state.
  • Demonstrated capacity for engaging in complex mental activities including strong judgment, articulate communication, calmness under pressure, empathetic listening, and flexible problem-solving.
  • Ability to prioritize tasks effectively and delegate responsibilities as necessary to deliver high-quality services in a demanding environment.
