• Provide psychosocial assessments and deliver tailored counseling to patients and their families, addressing their emotional, financial, and social needs.
• Utilize electronic medical records to meticulously document patient progress toward therapeutic goals and update the social work intervention plan as necessary.
• Collaborate with an interdisciplinary team, ensuring effective communication to anticipate patient needs and facilitate the overall plan of care.
• Support patients and families in understanding and executing transition plans, aiding in smooth transitions between different levels of care.
• Maintain updated knowledge of government and private payer resources to assist in patient transitions and coordinate care effectively.
• Act as an advocate for patients' rights, helping them access protective services and ensuring their voices are heard in the care process.
• Implement tools and technology solutions to identify patients at risk for readmission, developing proactive interventions as necessary.
• Participate in transition conferences with patients and family members, providing comprehensive support and resources.
• Report cases of vulnerability or suspected abuse as required by law to safeguard patients.
• Contribute to organizational goals by engaging in initiatives aimed at reducing readmission rates and enhancing patient outcomes.
• Completing other duties as assigned to support the mission of the organization.

In this role, you will support a diverse patient population that includes individuals facing acute and chronic mental health conditions, as well as families navigating the complexities of terminal illnesses. Your expertise will directly impact their journey towards recovery, equipping them with coping strategies and psychological support crucial for their well-being.

• A Master’s or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
• Possession of an independent license such as LCSW, LMFT, or LPC in the state of Oregon.
• Demonstrated ability to effectively manage a large caseload while providing quality care and maintaining individual patient focus.
• Familiarity with electronic medical records systems and efficient communication via email.

• Experience working in telehealth or remote counseling environments is highly desirable.
• Previous involvement in community programs that focus on mental health education and advocacy.
• A history of collaboration with interdisciplinary teams to achieve enhanced patient outcomes.

The position offers a competitive annual salary ranging from USD 100,000 to 105,000.
