• Conduct comprehensive clinical assessments and develop individualized treatment plans that consider the unique challenges and needs of each patient.
• Administer evidence-based therapeutic interventions, employing solution-focused techniques tailored to address the emotional and psychological aspects of our patients' experiences.
• Maintain accurate documentation and clinical records in compliance with organizational policies and state regulations, ensuring continuity of care.
• Collaborate closely with a multidisciplinary team, including other healthcare professionals, to create holistic treatment plans that support both patients and families during the hospice process.
• Provide education and ongoing support for patients and their families, facilitating understanding of the emotional and practical implications of hospice care.
• Stay abreast of the latest research and best practices in the field of social work and mental health to ensure the delivery of high-quality care.

In this role, you will primarily work with terminally ill patients receiving hospice care, as well as their families. Your expertise will play a crucial role in helping them navigate complex emotions such as grief, anxiety, and existential questions associated with end-of-life care. By providing empathetic support and practical assistance, you will significantly contribute to improving their overall quality of life during this challenging time.

• Master's degree in Social Work from an accredited institution.
• Valid LCSW license in the state of Michigan.
• Strong knowledge of solution-focused therapy and other evidence-based practices relevant to mental health.
• Excellent verbal and written communication skills, with the ability to connect with individuals from diverse backgrounds.
• Strong problem-solving abilities and a compassionate disposition.

• Previous experience in hospice care or palliative services is highly desirable.
• Familiarity with telehealth services and digital platforms that support remote therapy sessions.
• Experience in a multicultural setting, understanding the implications of cultural differences on patient care.
