• Conduct thorough clinical assessments utilizing our telehealth systems to understand the unique needs and circumstances of each patient, developing individualized treatment plans that align with best practice guidelines.
• Implement evidence-based therapeutic interventions, including but not limited to Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), ensuring that techniques are effectively adapted for remote delivery.
• Maintain meticulous clinical documentation through secure electronic health record (EHR) systems, ensuring compliance with state and federal regulations, and contributing to quality assurance processes.
• Engage in regular communications and collaborative case discussions with a multidisciplinary team, including psychiatrists and psychologists, to orchestrate comprehensive care tailored to each patient’s needs.
• Facilitate virtual educational sessions for patients and families to enhance their understanding of mental health, treatment processes, and benefits of various community resources.
• Stay current with ongoing developments in mental health care practices through participation in webinars and virtual conferences, coupling this knowledge with practical application in your ongoing clinical work.
• Manage a full caseload of remote clients, working diligently to ensure that treatment goals are met in a timely and efficient manner.
• Adhere to the ethical standards and guidelines set forth by the National Association of Social Workers (NASW) to maintain the highest level of professional integrity.

This role centers around patients within the hospice care environment, which means a focus on individuals who are facing terminal illnesses and their families. You will work compassionately to assist them in navigating emotional challenges, address grief, and enhance their overall wellness during significant transitions in life.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in Connecticut.
• Minimum of 2 years of clinical experience, demonstrating expertise in providing psychotherapy services in diverse settings.
• Proficient in employing solution-focused psychotherapy techniques with the ability to effectively adapt methods to suit remote practice.
• Strong communication and interpersonal skills to foster rapport with clients over virtual platforms, creating a trusting therapeutic relationship.
• Exceptional problem-solving and critical thinking skills, especially in overcoming challenges specific to remote care delivery.
• Capable of both independent work and effective collaboration within a virtual team environment.

• Experience working with palliative care or hospice patients would be a significant asset.
• Familiarity with telehealth technologies and electronic health record systems to optimize patient care.
