As a Licensed Clinical Social Worker, you will conduct assessments, therapy sessions, and care coordination from the comfort of your home, utilizing a variety of digital tools and methods. In this role, you will:

• Provide direct clinical services to assigned clients experiencing symptoms of major mental illness and severe emotional disorders, conducting both virtual and telephonic therapy sessions.
• Build and maintain trusting relationships with clients and their families, ensuring their voices are heard and integrated into their care plans.
• Conduct brief, evidence-based screenings to assess needs and determine appropriate levels of intervention.
• Create and implement integrated care plans while ensuring continuity of care by coordinating with various professionals, facilitating information exchange among clients, family members, and service providers.
• Monitor client progress during treatment and modify interventions as needed to adapt to the evolving needs of your clients.
• Participate in on-call coverage during and after program hours as assigned, offering crisis intervention when necessary.
• Complete timely reports and data submission to ensure compliance with regulatory and organizational standards.

Your primary focus will be on underserved populations within the Reading, Pennsylvania area. These clients often face barriers to accessing care, and your role will be pivotal in reducing these barriers through compassionate and effective mental health support. You will be working with clients across various backgrounds and cultures, which requires sensitivity and adaptability to different beliefs and traditions.

The ideal candidate will possess the following qualifications:

• A Bachelor's Degree in Social Work or a related field.
• A current, valid LCSW license.
• A minimum of two years of relevant clinical experience, preferably in a community health or mental health setting.
• Strong interpersonal skills and a commitment to fostering meaningful relationships with clients and colleagues alike.

While not required, the following experiences are preferred:

• Experience working with diverse populations, particularly in community or healthcare settings focused on treating major mental illnesses.
• Familiarity with integrated care models and trauma-informed care practices.
• Proficiency in using electronic health record systems and digital platforms for therapy delivery.

We offer a competitive salary range of USD 70,000 to 85,000 yearly, commensurate with experience.
