  • Lead discharge sessions that equip patients and their families with vital coping skills, transition plans, and appropriate resources to facilitate their ongoing recovery.
  • Confirm discharge appointments with patients and their loved ones, reinforcing support systems before exit from care.
  • Maintain ongoing communication with referral sources and outpatient providers throughout the admissions and discharge process to support continuity of care.
Patient Population / Client Focus
