Job description

Description

Location: Remote - Must reside in Georgia or a Central or Mountain Time Zone state
Schedule: Full-Time, Monday through Friday

Company Summary

Founded in 2021, Ascend Medical's mission is to elevate body, mind, and soul. We deliver advanced primary care that clinically integrates behavioral, metabolic, and lifestyle health to keep patients well physically, mentally, and emotionally.

Ascend began as a hybrid care model spanning digital and in-home services. We've since evolved to partner with traditional primary care providers, expanding access through brick-and-mortar, digital, and at-home channels.

We're built to innovate. That starts with culture. We avoid micromanagement and empower our team to make thoughtful decisions guided by our core values:

  • Patients before Profits
  • Empathy before Expedience
  • Progress before Stagnation
  • Team before Individual
  • Safety before Everything

We offer a competitive benefits package including:

  • Medical, Dental, and Vision
  • 401(k) with company match
  • Generous  PTO, paid parental leave, and company holidays
  • Free healthcare through Ascend, including telemedicine
  • Flexible scheduling and strong work-life balance

Position Summary

Ascend Medical is hiring a Full-Time Health Concierge with a primary focus on Senior Care services, including support for Assisted Living Facility (ALF) patients.

This is a remote, call center-based role supporting inbound patient inquiries generated through digital campaigns, community outreach, and referrals. You'll guide patients and families from their first interaction through appointment scheduling, portal setup, and preparation for a successful first visit.

Because this role supports senior care and ALF patients, we are seeking candidates with a healthcare background. Certification as a CMA or higher clinical credential is strongly preferred.

To support extended coverage needs, we are prioritizing candidates located in Georgia or states within the Central or Mountain Time Zones, allowing for later-hour scheduling flexibility.

What You'll Do

  • Serve as the first point of contact for senior care patients and their families
  • Schedule new patient appointments via phone and future digital channels
  • Verify insurance benefits and clearly explain patient cost responsibility
  • Document patient information accurately in the EHR and Salesforce
  • Communicate effectively with clinical teams regarding patient needs
  • Educate patients and families on Ascend services and how to access care
  • Provide compassionate, high-touch service tailored to senior populations
  • Support scheduling needs for Assisted Living Facilities, including later-hour      coverage when needed

What We're Looking For

This role is a strong fit if you have:

Healthcare Experience

  • Clinical background required; CMA certification or higher strongly preferred
  • Experience working with senior populations, assisted living, or primary care is      highly valued
  • Familiarity with insurance verification and healthcare systems

Call Center or Patient-Facing Experience

  • Comfortable managing a high volume of inbound calls
  • Customer-focused with strong communication skills

Strong Technical Skills

  • Proficiency in Microsoft Outlook
  • Experience with EHR systems and Salesforce preferred

Work Style

  • Self-directed and accountable
  • Organized and detail-oriented
  • Calm, empathetic, and solution-focused
  • Reliable for full-time coverage needs

Education

  • High School diploma or equivalent required
  • Bilingual Spanish is a plus
